Kim Kelly, RN, BSN, LNC - CEO & Principal Consultant
Kim is the founder of Compliance Review Services, Inc. and began the company in 1997. She graduated from Ohio State University with a Bachelor of Science in Nursing in 1986. She has 20+ years of experience in the healthcare field, ranging from Administrator, to Director of Nurses, to home health agency owner. She has worked in multiple medical environments, taught at a vocational college, and developed and taught numerous educational offerings. Kim is a member of the Home Health Alliance Team, Texas Association for Health Care, and DADS external stakeholder group for rule review. She is a Texas Department of Aging and Disability Services Consultant-Monitor and currently sat on the Texas Outcome Based Quality Management coalition team. Ms. Kelly recently became an independent contractor for DADS conducting long term care quality reviews. Ms. Kelly became a Legal Nurse Consultant in 1999. She has been a consulting nurse expert and testifying nurse expert witness in both plaintiff and defense cases. In addition, Kim was inducted into the Business Who's Who in March of 2004 for her accomplishments as a small business owner. In addition, she is an honorary chairperson for small businesses through NRCC and is an external workshop member for DADS for the State of Texas. See her complete resume (Microsoft Word).

Outside of work, Kim enjoys spending time with her family. An accomplished pianist, she also likes swimming, reading, bowling, and watching movies. Kim is originally from Cleveland, Ohio and resided in Florida for several years. She has lived in Texas since 1995.

Michael Bradshaw, RN, Assistant Senior Consultant

  • Michael earned an Associate degree in Nursing in 1977 from Angelina College in Lufkin, Texas.
  • His nursing career began in a hospital setting including: Medical/Surgical Staff Nurse, Shift Supervisor, Assistant Charge Nurse, Intensive Care Staff Nurse, Intensive Care Manager; Home Health and Nursing Home which included Administrator, Supervising Nurse, QA Nurse, Assistant Director of Clinical Services, Home Health Aide Supervisor, and Field Nurse.
  • Michael worked as a HCSSA Nurse Surveyor for the Department of Aging & Disability Services in Houston, Texas for 5 years before joining the Compliance Review Services, Inc. team in 2005.

    Janice M. Johnson-Umezulike, RN, BSN, MN, CNS, DNS - Educational Consultant
    Dr. Johnson-Umezulike received her Bachelor of Science in Nursing from McNeese State University in Lake Charles, Louisiana, in 1984. She received both her Masters in Nursing and Doctor of Nursing Science from Louisiana State University Medical Center in New Orleans, Louisiana in 1989 and 1997, respectively. Since 1990, she has been a member of Sigma Theta Tau, International Honor Society of Nursing and many other professional organizations. She also serves on the Board of many professional organizations and has published a variety of nursing articles. Janice is currently a professor and continuing education instructor at Lee College in Baytown, Texas Her specialty areas are in Medical/Surgical Nursing, Education, Orthopedics, Pain and Pain Perception, Curriculum and Teaching, and Test Taking Consultation.

    Since joining Compliance Review Services, Janice has submitted many educational activities to the Texas Nurses Association in which all activities were approved by the TNA. Her main role in the company is to write educational activities so that all activities can be approved by the Texas Nurses Association.     Marie Luque - Office Manager

  • Marie worked for many years in human resources for the state of New York before marrying into the oil business and traveling the globe. She lived overseas for 15 years in Asia and South America, with two daughters, Joline and Jessica, born in Bolivia and Chile, respectively. After she returned to the U.S., Marie worked in real estate marketing for several years from her home office. She joined the Compliance Review Services, Inc. team in April 2004.
  • As a frustrated English major, Marie has been writing for some time and has had a short story published. Her goal is to have one of her two novels in shape to submit for publication within her lifetime. Marie is also devoted to soccer and is a player in an over-30 league. Her other hobbies include golf, squash, tennis, and volleyball.

    Jayne Jessop - Referral Coordinator
    Jayne Jessop joined the Compliance Review Services, Inc. Administrative Team in July 2006. Ms. Jessop has over 15 years of sales and diversified administrative experience, and has wasted no time becoming an integral part of the Compliance Review Services, Inc. family. Ms. Jessop comes to us via England, and has put her experience both here and abroad to work in helping Compliance Review Services, Inc. acquire new business as well as give our existing clients, managers, and consultants the highest level of service and support.

    Kristen Ramirez - Administrative Assistant
    Kristen Ramirez began with Compliance Review Services, Inc., in October 2005. We are happy to have her on board as Administrative Assistant. Kristen will be assisting in the customization and production of our start up packages, handle marketing and seminar materials, and coordinates consulting schedules with the Assistant Senior Consultant. Kristen has a daughter, Gabby and a son, Brayden and enjoys spending her free time with them.

    Barbara Rodriguez - Administrative Assistant
    Barbara joined the Compliance Review Services team in October of 2006 and quickly moved to full-time permanent status as of November 2006. She comes to us with an impressive customer service background which includes extensive experience with computers and account maintenance. Barbara is also a veteran who served our country in the military for 12 years. Barbara’s duties include receptionist as well as coordinating our many education on-site offerings..

    Brooke Paul - Administrative Assistant
    Brooke Paul is the newest member of our Compliance Review Services, Inc. team, joining us in November, 2007. She has over 3 ½ years of customer service experience, most recently with Chase Bank where she worked in the Operations Department. Brooke is our front line employee answering your calls and providing direction and friendly support. In her down time, she enjoys decorating, scrapbooking, jogging, poetry writing and spending time with her family. She and her fiancé, look forward to their marriage and future plans for their own family.

    Joyce May, RN, BSN, Nurse Consultant

  • Joyce began her nursing career in 1972 at the first NICU in the Dallas area. After helping to open the unit and serving as Assistant Head Nurse, Joyce moved to Big Spring, Texas, in 1974 and was Charge Nurse PACU/Staff Nurse OR in a hospital. From 1976 to 1984, Joyce wore multiple hats at an Andrews, Texas, hospital, and served as Charge Nurse/Assistant Director OR/PACU, Staff Nurse OR/PACU, and Director of Material Management Department. She developed the procedure manual for the department, was the quality assurance representative for OR/PACU, and was active on the Safety and Infection Control committee. After two other hospital staff nurse positions, Joyce became a Nursing Specialist for the Texas Department of Health/Department of State Health Services. From 1990 to 2004, she conducted initial and recertification surveys, evaluated healthcare agencies for compliance with Medicare Conditions of Participation and State License Rules, and provided educational and consultative assistance to all provider types. In 2005, Joyce joined the Compliance Review Services, Inc., team.
  • Joyce obtained her Associate in Applied Science from El Central College in Dallas, Texas, in 1972 and her Bachelor of Science in Nursing from Texas Tech University in Lubbock, Texas, in 1989. She received nine months of training in the Texas Department of Health Continuous Quality Improvement process in 2001 and her CLEAR Certification for Complaint Investigation in 2003.

    Christy Slaughter, RN, BSHA, MSHP, LNC, HCS-D, Nurse Consultant
    Christy began her career at Midland Memorial Hospital in 1979. She graduated from Odessa College in Odessa, Texas, with an Associate of Nursing degree in 1980 and worked in staff and charge nurse roles at the hospital until 1990. After that, she worked for a series of home health care companies in positions ranging from staff nurse to Patient Care Coordinator to Assistant Director of Professional Services. In 1997 and 1999, Christy earned her Bachelor and Master of Science degrees from Southwest Texas State University in San Marcos, Texas. She became certified as a Legal Nurse Consultant in 2002 and a Homecare Coding Specialist - Diagnosis in 2004. Christy has been working for Compliance Review Services since 2000.

    Linda Henslee, COS, Nurse Consultant, Educational Consultant/Trainer, Field Consultant
    Linda Henslee comes to Compliance Review Services, Inc. with many years of Home Health experience, as an Administrator, Alternate Administrator, QA Coordinator, and as a Nurse Consultant. Ms. Henslee attended nursing school and received her degree in Arkansas. In addition to her years of experience as a staff nurse, Ms. Henslee joined our consultant team in July 2006, and has proven uniquely qualified to give our clients the best possible consulting services. Linda is a Certified Oasis Specialist and is also a JCAHO consultant who is very well versed in JCAHO standards. Ms. Henslee will be presenting classes for the consulting firm and assisting in training newly hired consultants.

    Denise Garcia, RN Consultant
    Denise Garcia, RN earned her BS in Nursing from Mercy College of Detroit, in Michigan and continued with an MS in Health Services Administration from St. Joseph's College in Standish, Maine. Denise has 20 years of experience ranging from Director of Operations for a multimillion dollar non-profit home health agency, to Administrator of two home health care operations, and including positions as Patient Care Administrator, Director of Clinical Services and Director of Nursing in the area of Hospice. We are proud to have Denise bring her expertise to our nurse consultant staff as of August 1, 2007.

    Paula L. Frate RN, HNC
    Paula began her nursing career in 1978 with an Associate Degree of Applied Science in Nursing from a small college in northern Ohio. She relocated to Houston, Texas in 1981 and comes to Compliance Review Services with a broad background in both business and healthcare.

    Her clinical experience spans the continuum of healthcare including intensive care, emergency room, rehabilitation, home health and hospice. In 2000 she became certified as a holistic nurse to better address the needs of dying patients and their families.

    Paula's business experience began in 1984 as a consultant to insurance companies and large corporations implementing case management and health promotion programs. Over the past ten years she has worked with a large county hospital system to improve charge capture and revenue management for medical research and as an Education and Performance Improvement Coordinator with a large home health and hospice agency. She is an experienced home health and hospice software trainer and is currently pursuing certification as a business coach.

    Stephanie Worthen, RN
    Stephanie Worthen graduated from the University of Texas Medical Branch in Galveston with a B.S.N. in 1979 and a M.S.N. in 1980. Stephanie has 20 years of home health experience with recent experience in operations of a stand alone proprietary hospice. Stephanie was a surveyor with the Texas Department of Aging and Disabilities Services for four years. Stephanie is also a Legal Nurse Consultant with recent experience in medical malpractice. Stephanie developed several power point presentations for her last employer that were utilized across multiple states to educate nurses on such topics as Management and Evaluation of the Patient Care Plan and Cultural Customs in the care of dying patients.

    2.   Review of medical records to identify failures to the standards of care
    3.   Prepared testifying expert reports
    4.   Prepared 4590i reports
       5.   Prepared settlement tables
       6.   Prepared graphics
       7.   Prepared time lines of events, significant issues
       8.   Prepared case summaries
       9.   Assisted in writing interrogatories for physicians and nurses.

    Kim has worked as a nurse consultant with trial preparation including::

    1. Identifying and choosing key source documents to be demonstrative evidence
    2. Assisting with pulling appropriate authoritative text information for demonstrative evidence
    3. Assisting with pulling regulations and standards of care information for demonstrative evidence
    4. Chronologies for key source documents based on each experts testimony
    5. Reviewing opposing counsel depositions and assisting with identifying incorrect information, rebuttals to their theories
    6. Assisting with gridding of failures, statistical analysis
    7. Assisting with creating graphics for information specific to the case, including meeting with graphics personnel.
    8. Writing direct interrogatories and cross examination interrogatories
    9. Educating attorneys on failures to care, documents that are relative to this, and clinical issues impacting the case.

    She is currently engaged with multiple law firms for both plaintiff and defendant cases.

    Bette Lou Hurford, RN, RNC, CRRN, LNC

  • Bette Lou became a registered nurse in 1964 after her graduation from the Henry Ford Hospital School of Nursing in Detroit, Michigan. Her background includes hospital and long-term care nursing in Michigan and Florida, and she has held both staff nurse and supervisory roles, such as Director of Nursing and Assistant Director of Nursing. She became certified in Rehabilitation in 1990, in Gerontology in 1997, and as a Legal Nurse Consultant in 2004 through 2006.
  • Since joining the Compliance Review Services team in 2002, Bette Lou has worked on legal cases involving nursing home and hospital cases. She has reviewed medical records to identify any deviations from the standard of care, reviewed cases to determine if they are meritorious, prepared 4590i documentation, Chapter 74 reports, assisted with settlement tables, identified key source documents, and assisted with trial preparations.

    Lindsey Gregory Tyner - Website Developer
    Lindsey Gregory was born in Sugar Land, TX. She moved to Austin to attend and graduate from the University of Texas with a degree in Radio-Television-Film. Lindsey has worked as a web developer, graphic designer and user interface engineer for almost a decade. She has worked in an array of industries including healthcare, hospitality, and software industries.

    In her spare time, Lindsey enjoys the live music scene in Austin and travelling.

    Edward P Hughes - Technology Consultant
    Ed has been in the Information Technologies industry for the past 23 years. He has held positions from Software Developer to Project Manager. He also has, owned and worked for a number of computer consulting companies.

    Ed lives in Cleveland Ohio. He has 2 sons and loves spending as much of his free time with them.
